Family & Health Leadership Contract

 

Agree and Sign the “Health Leadership Contract”, “Family Leadership Contract”, PLUS complete “Are You Sleeping”. Submit all three documents to AAW and enter to win a $5.00 Target Gift Card.  There is a $5.00 winner every month !

Every month, I will review this contract to make sure I uphold all my above freedoms and responsibilities. I recognize that freedom is a responsibility to myself, my family, and my neighbor.

As a valuable member of a family unit by my own definition, I recognize that I own the power to good health and happiness. By reading and signing this family leadership contract, I commit myself to the following freedoms and responsibilities that enable me to live well.

  1. At any age, I have the freedom to choose to eat nutritious foods that will protect my body from disease.
  2. At any age, I have the freedom to choose to have relationships with positive people who respect me for my character and intelligence, and will not harm me mentally of physically.
  3. I have the freedom to choose to NOT smoke, to NOT drink too much, and not to take street drugs.
  4. When the time is right, as a financially stable, mature man or woman, I have the freedom to choose to have a healthy pregnancy, with a healthy partner, to make sure I start a healthy family.
  5. I have the freedom to choose the spiritual guidance that keeps me centered, and helps me to make reasonable, positive decisions for my personal and family happiness.
  6. I have the responsibility to take the needed steps to become a positive member of my family unit. These needed steps include: achieving a good self esteem, getting the best education of my potential, communicating and sharing responsible ideas and feelings.

A family leader is someone who, at any age, has taken the steps to becoming a positive member of their family unit. I am a family leader with all the human rights and potential given to me at birth.

  1. I am a family leader. I define my family members to be those individuals I love. With my health, and the health of those I love in mind, I commit myself to the following freedoms and responsibilities that enable me to live well.
  2. I have the freedom to be responsible for my good health by eating nutritious food, not drinking too much alcohol and not smoking.
  3. I have the freedom to live free of street drugs and use prescription drugs sensibly.
  4. I have the freedom to live, work, and study in a clean, drug free environment.
  5. I have the freedom to seek the highest level of education I choose to benefit myself and my family.
  6. I have the freedom to choose high self-esteem and always feel good about myself.
  7. I have the freedom to choose to surround myself with others who too have high self-esteem and feel good about themselves.
  8. I have the freedom to choose to have a healthy pregnancy as a responsible woman.
  9. I have the freedom to choose to have a healthy pregnancy as a responsible man.
  10. I have the freedom to choose to build a healthy family and a healthy community.
  11. I want to live powerfully by staying healthy. Signing this contract shows that I am committed to being physically, spiritually, and mentally healthy.
  12. Every month, I will review this contract to make sure I uphold all my above freedoms and responsibilities. I recognize that freedom is a responsibility to myself, my family, and my neighbor.
